"It's just how you really are inside"

It isn't always easy for Jacob to explain his identity to people he meets. "Most of the people aren't bad people. They just don't really understand," explains Jacob, a nine-year-old transgender child.

NBC first interviewed Jacob when he was just 5. Now, as he approaches puberty, Jacob and his family face a new set of choices.

They don't know exactly what lies ahead, but Jacob's parents are firm in their support for their son. "There is no harm in saying to your child, I see you and I believe you and you are who you say you are," says Jacob's mother, Mimi.

Watch this latest interview by NBC to catch up with Jacob and his family's story.
